Middle aged man walks to OTC counter asking for a ventolin. Can I get a Ventolin please he asks.

B) I have a dry cough that can get bad enough that I have trouble catching my breath. It is really bothering me at night

C) About a week ago with this change in weather

D) Just some butter menthol lozenges to try and help stop with my cough, they didn’t do much

F) Asthma but hasn’t used Ventolin a few months –> the one he went to use today was out of date

  1. Cough can be quite noisy sometimes, sounds a bit wheezy
  2. No its a very dry cough
  3. I do get coughs from time to time, usually, I use my Ventolin and it seems to work well

  6. I was given one by my doctor but didn’t use it much
  7. No, just my nasty cold

What is the diagnosis? Explain reasoning to patient.

A

Based on the signs and symptoms you have told me, I believe you have asthma. Symptoms such as the wheezing, history of asthma and having allergies to dust and pollen has confirmed it for me.

What product would you recommend for this patient? Provide IDARL.

A

Salbutamol (Ventolin)

  • Indication: Will relax your airways, help you breathe easier and suppress the need for you to cough.
  • Dosage and directions: Take the cap off and shake, breathe away from the device, form tight seal with lips, depress cannister, deep breath for 5 seconds, take the inhaler out and breathe away
  • Adverse effects: Tremor, palpitation, headache, tachycardia,
  • Duration of treatment, time to recovery, when to refer: use when required, will refer you to a doctor if you are using it 4-5 times a week, fever, chest pain, don’t have AAP, use it but never seen a doctor before.
  • Lifestyle/self-care: make you sure you have your inhaler with you at all times, wear a mask, reduce exposure to trigger
