Polyarteritis nodosa

A

systemic necrotizing vasculitis that typically affects medium-sized muscular arteries but unlike other
vasculitides is not associated with ANCA. May be associated with Hep B and Hep C. Clinical features include livedo reticularis, skin ulcers, tender erythematous nodules, bullous or vesicular eruption and/or ulcers. Renal involvement leads to hypertension and CKD.
Neurologic disease may include mononeuropathy multiplex. Muscle involvement is common including myalgia and muscle weakness, though not usually severe. ANCA should be negative. Biopsy demonstrates inflammation of medium sized arteries.

Giant cell arteritis treatment

A

Prednisone 40-60mg po daily x 1 yr, can do biopsy within 7 days of starting therapy.
ASA to prevent CVA/TIA
MRI/angiogram preferred diagnosis
