why do you use STR
It is generally accepted that the effects of massage and soft tissue release are short lived. So why do we bother?
Short term effect of increased flexibility of a muscle which can promote normal and better movements patters which are long lasting.
Good anecdotal evidence that it reduced pain, increase flexibility and alters movement patterns.
It can help with tissue desensitisation.
There are many other benefits to hands on treatment – mentioned in later slide.
what are the benefits of hands on contact
Research has demonstrated that touch and hands on contact can:
Increase feelings of affinity with another person.
Increase positive feelings in patients.
Increase oxytocin.
Reduce cortisol.
Reduce heart rate and blood pressure.
Improve bonds between patient and therapist which can translate into a patient’s greater engagement in treatment plans.
Has perceptions of increased care.
Increases the treatment affect compared to hands off treatment.
Oxytocin amongst other things is believed to have a significant role in social learning, social bonding and can act as an anti-depressant.
Cortisol is a stress hormone, released with long periods of stress. It affects your digestive system, immune system, sleep and more.
what are the barriers to hands on treatment
There are many barriers to hands on treatment., some are easier to overcome than others.
Clothing
Mild anxiety / apprehension
Touch aversion (why might people be touch adverse?)
Religion or beliefs
Pain
what are some precautions for soft tissue injuries
Sprains, tears and contusions (can lead to Myositis Ossificans)
Infections
DVT
Broken skin or open wounds
Patients with bleeding or clotting problems
Unidentified lumps / bumps
Anything else you are unsure of!
what is trigger point release and how do you conduct it
Palpate the muscle until you find a painful band or lump:
QL or Erector spinae.
Glutes
Calf
Apply direct pressure (with thumb or elbow) until the pain subsides.
It should be painful initially, then reduce to being sore, then reduce to minimal pain.
Note if pain refers anywhere.
how do you conduct Muscle energy techniques (METs)
Check muscle length before you start
Perform 3 x contract relax cycles
Take the muscle to the end of the available range
Ask the patient to gently push and resist their force for 7 seconds
Relax and wait a couple of seconds
Push the muscle to the new movement barrier.
Try with your plinths doing and MET on the calf, quads and hamstrings.
what are some examples of soft tissue injury contraindications
Open wounds – contraindication, contraindication, could make it worse or infect it
Myositis ossificans – contraindication, could be made worse while massaging
DVT – contraindication,
Infection – contraindication, nothing to be achieved by massaging as it needs treating
Bleeding / clotting problems – could bruise during massage/ more likely to bruise patient
Skin infection - contraindication
Unidentified lumps / bumps – contraindications,
what are some examples of soft tissue injury precautions
Allodynia – precaution,
Fragile skin – precaution, could break skin during massage
Pregnancy – precaution, ligaments and soft tissues, can affect pain, never any research due to ethical reasons
