A torsion is a twist in the spermatic cord which shortens the cord and so raises the testicle to a higher position. This soon cuts off the blood supply, so that the testicle rapidly becomes very painful. It is an emergency, and has to be referred immediately as the testis starts to show necrosis in only a few hours.
Epididymo-orchitis is inflammation usually caused by infection, and a recent urinary tract infection or instrumentation is common.
A hydrocoele on its own is usually non-tender. Note that reactive hydrocoeles can be generated by an underlying testicular tumour, torsion or epididymo-orchitis. The latter 2 conditions are painful.
Tumours are usually not painful.
