• Plans valid for 1yr from purchase date, $30 cancellation fee but waived within 72 hrs. of purchase, plans active more than 9mo. cannot be cancelled
• Plan must be intended for pet it was purchased for
• Must schedule appointment
• Services only provided by Mast Blvd Pet Hospital
• No double discounts
• Any additional deworming & GI PCR parasite tests will be at owner’s expense, tapeworm, coccidia & giardia treatment not included in plan
• E-collars for spay/neuters are are owner’s expense, additional charge for cryptorchids, pregnant or in-heat pets
• Extractions, pain injections, meds to go home & any additional surgery procedures are not included in plan prices
