Finkelstein’s Test - purpose
determine presence of de Quervain disease, a paratenonitis in the thumb
Finkelstein’s Test - description
pt makes a fist with the thumb inside the fingers. examiner stabilizes the forearm and deviates the wrist toward the ulnar side
Finkelstein’s Test - result
pain over the abductor pollicis longus and extensor pollicis brevis tendons at the wrist
Bunnel-Litter (Intrinsic-Plus) Test - purpose
identifies tightness in structures surrounding the MCP joint
Bunnel-Litter (Intrinsic-Plus) Test - description
MCP joint is held slightly extended while examiner move the PIP joint into flexion. pt remains passive during test
Bunnel-Litter (Intrinsic-Plus) Test - result
(+) inability to flex the PIP joint. there is a tight intrinsic muscle or contracture of joint capsule. if MCP joints are slightly flexed, PIP joint flexes full if intrinsic muscles are tight, but it does not flex fully if the capsule is tight
Froment’s Sign - purpose
identify ulnar nerve dysfunction
Froment’s Sign - description
pt attempts to grasp a piece of paper between the thumb and index finger
Froment’s Sign - result
(+) when examiner attempts to pull away the paper, the terminal phalanx of thumb flexes because of paralysis of the adductor pollicis muscle. if at the same time, the MCP joint of thumb hyperextends, (+) Jeanne’s sign. If both tests (+), ulnar nerve paralysis
Wartenberg Sign - purpose
identify ulnar nerve neuropathy
