Leiomyoma. Subserosal, submucosal
RF
Obesity, early menarche, increase age, fam hx, afrocarrib.
Presentation
Menorrhagia, pelvic pain, pelvic pressure, subfertility.
Investigations
Pelvic US
FBC
Pregnancy test
MRI if sarcoma suspected.
Outline medical managements
Tranexamic acid
HRT such as IUS Mirena coil or COCP/POP
GnRH analogue (zolidex)
Ulipristal
Both of these mainly for pre surgical
Surgical Management
Indicated if medical management no responses, pressure symptoms, XS uterine size.
Surgical myomectomy or hysterectomy. Can be abdominal or laprascopic.
Uterine artery embolisation
Complications
Iron defeciency, urinary incontinence due to increase pelvic pressure, problems during pregnancy (miscarriage, fetal PPH, prematurity)
