Aetiology

A

XS LH due to increase frequency pulses from GnRH, stimulants ovarian production of androgens

Insulin resistance leads to higher levels of adrogens as well.

Increase androgens means LH surge is supressed leading to follicle development to arrest and remain as cyst.

Presentation

A

Infertility, amenorrhoea, obesity, depression, hirsutism, pelvic pain.

Investigations/diagnosis

A

Two out three of -

Olig/anovulation
Biochem/clinical hyperadnrogenism
Polycystic ovaries on pelvic ultrasound

Management

A

Treat HTN and diabetes

Manually induce 3 bleeds every year with COCP or dydrogesterone

Clomifene (SERM) helps follicles but can have a chance of twins.

Hirsutism - Finasteride, spirinolactone.
