What indications are indicative of fuel clad barrier loss or potential loss?

A

Loss - CET > 1200 deg F, Containment high range monitor > 750 R/hr, RCS sample > 300 uCi/gm I-131

Potential Loss - RVLMS 1-9 dry, CETs > 700 deg F, RCS heat removal can’t be established using SGs and SRO determined HPI cooling conditions have been met

What indications are indicative of RCS clad barrier loss or potential loss?

A

Loss - ESAS actuation required by RCS leak or SG tube rupture, RB High range > 40 R/hr

Potential Loss - RCS or SG Tube leakage > 50 gpm excluding normal reducations in RCS inventory (letdown, RCP leakoff, etc), PTS limits apply and RCS pressure/temp are left of NDTT limits in Fig 3, RCS SG heat removal cannot be established and SRO determines HPI cooling necessary.

What indications are indicative of containment barrier loss or potential loss?

A

Loss - SG leakage > 50 gpm or is RUPTURED (requires ESAS) is also faulted outside containment, Containment isolation required and unisolable path from containment to environment exists or has been lost based on ED judgement, Indications of RCS leakage outside containment.

Potential Loss - CETs > 1200 deg F and restoration procedures not effective within 15 min, Cont high range monitor > 10,000 R/hr, Cont pressure > 73.7 psia, Containment H2 conc > 4%, Containment pressure > 44.7 psia with < 1 full train of containment heat removal > 15 min.
