What are immediate actions and contingencies for RX trip PB?

A

1) If RX fails to trip depress backup CRD power supply breaker PBs on C03 (A-501 and B-631)
2) If backup PBs fail, Manually insert rods and send operator to open CRD breakers
3) If more than one rod fails to FULLY insert or power not dropping perform RT-12

What are the immediate action contingencies for the turbine?

A

1) If 125V DC Bus D01 de-energized by turbine trip solenoid light out and breaker position indication for A3 and odd bus 480v on C10 and either D01 undervoltage alarm or D01 < 105v locally then transfer D11 to emerg supply D02 on C10 and perform loss of D01 AOP in conjunction.
2) If either SG < 900#, actuate MSLI for affected SG, actuate EFW, check SCM adequate and perform RT-6 (EFW and MSLI). Go to Overcooling or LOSCM.

What steps are performed if bus A3 or A4 is de-energized?

A

1) Check for LO relay trip alarm
2) Check EDG running (RT-21) -> If not running then investigate and place AAC generator on bus using 1107.002.
3) Reset DG output breaker anti pump feature by taking to PTL if diesel running and output breaker open. If EDG output breaker won’t close then check A1/A2 energized and attemp to close A-309/A-409 using sync switch.

What actions are performed if A1 or A2 de-energized and needed?

A

1) Check LO relay alarm clear.
2) Check SU1 voltage >/= 22KV
3) Energize dead bus from SU1 using sync switch. If fails to close reset anti-pump.
3) If unable to feed from SU1, energize A1/A2 from SU2 using Att E of 1107.001 for loading and energize loads as necessary.

What actions are performed if Turbine Bypass valves not controlling 950 - 1020 psig?

A

If TBVs failed closed, then

1) If vacuum > 23”, depress low vacuum reset PBs on C02 and verify TBVs operate in hand and place them in auto.
2) If Vacuum < 23” or TBVs fail to work after PBs depressed, verify ADV isolation open and ADVs controlling 1000 - 1040 psig and check for tube leakage on MS High range monitor.

A

1) Place ICS override switch in override and close block valve. If fails then actuate EFW and close the associated loops MFIV. Perform RT-5.

A

If fails to close, then

1) Close LL valve

If LL fails to close

1) Actuate EFW, Close associated MFIV and perform RT-5.

If fails to open,

5) If RCS pressure < 1550 psig Go to ESAS.

A

1) Operate CV-1235 in hand and control at 90 - 110” if not responding in auto.
2) If < 55” and no ind of recovery, isolate letdown.
3) Ensure PZR heaters off.
4) If < 30” then initiate RT-2.

How is PZR steam space integrity checked and what actions are taken if it is not adequate?

A

1) Verify PZR spray valve closed
2) Verify ERV isolation closed
3) Use all available PZR heaters to maintain RCS pressure
4) If PZR level continues to rise without rise in temp or pressure then re-open CV-1000, If RCPs off restart one RCP in each loop using RT-11 or GO TO Natural Circ Cooldown if RCPs not available. If RCPs running GO TO Forced Flow C/D.

What actions are performed if no RCPs are running?

A

1) Verify EFW (RT-5)
2) If unit 2 has loads on SU2, then do not start RCP if unit 1 aligned to SU2 due to potential for millstone relays to actuate.
3) If H1 or H2 energized and SCM adequate and RCPs available, start one RCP per loop (RT-11)
4) If RCPs unavailable and cooldown to be performed, perform Natural Circ C/D in conjunction
