exclude fever + obvious infectious cause - neutropenic sepsis

aggressive rehydration - 4-6L 0.9% NaCl in 24hrs

IV bisphosphonate = 4mg zoledronic acid over 15 mins

(2nd line: prednisolone, calcitonin)
