Steering Gear failure

A
  1. Check all modes of steering
  2. Check both steering pumps
  3. Inform Master
  4. Inform engine room
  5. Take way off ship
  6. Engage emergency steering
  7. Prep Bow thruster, engines for manoeuvre
  8. Verify position, check for hazards, traffic situation, consider anchoring
  9. Consider exhibiting NUC, Commence sound signal D
  10. Update VTS, VHF

Main Engine Failure

A
  1. Inform master
  2. Maneuver the ship away from immediate danger
  3. Chart assessment
  4. Prepare for anchoring if in shallow water
  5. Exhibit NUC lights/shapes
  6. Broadcast urgency message, if appropriate
  7. Update AIS
  8. Inform VTS, etc.

Initial Action Following: Collision

A
  1. Sound general emergency alarm
  2. Call the Master
  3. Stop main engines in most cases, manoeuvre as best to reduce damage
  4. Close all watertight and fire doors
  5. Start bilge pumps and fire pumps
  6. Muster all crew and passengers. Check for casualties or missing persons
  7. Assess the extent of the damage
  8. Consider additional factors, such as the risk of fire
  9. Position on chart established and safe port options
  10. Offer assistance to other ship
  11. Sound all tanks
  12. Initiate SOPEP if required
  13. Depending on the amount of damage and general circumstances, decision to
    abandon or not to abandon will be taken
  14. Send appropriate radio messages (mayday/urgency)
  15. Display appropriate lights or shapes
  16. The Master should report the casualty as soon as practicable. This report
    should be made to the Marine Accident Investigation Branch (MAIB).

Initial Action following: Grounding

A
  1. Stop engines
  2. Close watertight doors
  3. Inform Master
  4. Sound general alarm: Muster crew/passengers and check for casualties
  5. Check for hull damages and assess internal damage by visual inspection where
    possible
  6. Sound Tanks, Look for signs of pollution initiate SOPEP
  7. Exhibit lights and shapes for aground
  8. Sound for available depth of water about the vessel, especially around stern
    and propeller area
  9. Check position of grounding on chart. Determine the nature of the bottom and
    expected depth of water
  10. Determine if wind and sea are carrying the vessel harder aground
  11. Determine state of tide on grounding, together with heights and times of the
    immediate high and low waters
  12. Instigate temporary repairs to reduce the intake of any water, activate bilge pump
  13. If assistance is required, transmit a mayday or pan pan.

A

Broadcast Distress msg on Masters Authority
Instruct crew to don lifejackets and immersion suits as appropriate
Must all crew at lifeboat stations
Prep LSA for Launch
Collect EPIRB, SART, SOLAS radios, grab bags
Launch and embark LSA
Ensure lifeboats and rafts stay together and clear of Ship
Acivate Sart and Epirb
