What is the 30 day rule for FPDR?
Applications for disability benefits must be submitted no later than 30 days
What is the 14-day rule for FPDR?
100% sick leave reimbursed if claim filed within 14 days after injury. After 14 days, it’s 75%
The Personnel System will automatically email a notice of Injury report to who?
BATTALION CHIEF
Upon completion by BC it also then automatically sends a notification to BHQ and SAFETY CHIEF
What are the Company Officer responsibilities for Injury Recording? (7)
- Report to on-Duty Battalion Chief immediately (who was on-duty at time of injury)
- Complete PERSONNEL System Injury report
- Enter injury in JOURNAL System
- provide PURPLE packet
- Assist member in completing DILD, then forward to Battalion Chief
- Notify BHQ (when notified of ETA off/coming back)
- WSR forward to BC (when received
