Describe 5 nursing assessments before bringing patient to the toilet.
-assess the needs to assist patient with toileting
-assess patient’s current mobility lvl
-assist patient’s ability to assist in positioning the elimination device
-Assess the need to collect any urine specimens prior to elimination or discarding waste
-Assess the need to document patient’s output in the elimination chart before discarding waste
Explain 5 safety measures with rationale when using a commode at the bedside.
-Always ensure that the call bell is close to patient’s hand if patient is allowed to be left alone
-Assist patient with the use of toilet paper or wet wipes
-If a patient is using commode, ensure commode is placed close to the bed or chair with breaks applied
-access the patient’s mobility and provide assistance to move patient onto the commode if required
-report and record observations in intake and output chart
- Explain 5 principles related to safety, standard precautions, and body mechanics when assisting patients with the use of elimination devices, support the answers with rational.
-Consider patient’s mobility when assisting during elimination
-Ensure there are the necessary supplies at the bedside to allow smooth and ease of access
-Commode should be placed close to the bed
-All equipment should be in working condition
-Encourage patient to perform hand hygiene after their elimination
