Initial Cardiac Resuscitation
A
- Check bewustzijn
- Niet reageren? -> check pols en AH
- Act reanimatiesein, sluit aan op AED/monitor en start BLS. >100/min, 5cm diep. 30:2
- Defibrilatie asap bij VF of VT. Mono 360J, bi 200J. Direct daarna 2 min BLS
- Geintubeerde patient DIRECT van machine af en op ballon (100%).

Delegeren van taken
A
- Vrije luchtweg. Ballon of ET, 8-10/min (ivm positieve druk voor coronaire perfusie)
- Borst compressies. >100/min, continue bij gezekerde luchtweg.
- ECG/monitor + evt echocardio
- I.V. I.O. toegang + lab (ABG, glucose, electrolyten, kalium, calcium, fosfaat, magnesium)
- Medicatie
- Bijhouden
- Patient status
- Hoogte brengen familie, creeeren ruimte (onnodig personeel)

Targeted Temperature Management (TTM)
A
- 32-36 graden
- Verbeteren neurologische uitkomst en – mortaliteit
- Ten minste 24h
- Bijwerkingen: stollings, verhoogd infectie, arrythmia, hyperglycemie
TTM protocol
- Start ASAP
- Extern of intern koelen
- Adequate temp meting
- Normo alles: pH, pCO2, glucose, electrolyten
- Perfusie: behandel SBP <90 en MAP <65
- Behandel insulten en rillen

Mechanically ventilated Cardiac Arrest
A
- complicatie van beademing: (spannings)pneumothorax, ventilator falen/disconnectie, obstructie/dislocatie
- ONMIDDELIJK loskoppelen en ballon 100% O2
- Spanningspneumothorax (hypotensie, PEA+small complex VT, CVD+) –> 16-18 holle naald 2e IC mid-claviculair (ter plaatse van afw ademgeruis of subcutaan emfyseem)
- Direct vervangen voor thoraxdrain
