3.4.2 Minimum Temperature for Criticality

A

Each RCS loop average temperature (Tavg) shall be ≥ 551°F.

APPLICABILITY: MODE 1 and MODE 2 with keff ≥ 1.0.

3.4.3 RCS Pressure and Temperature (P/T) Limits

  • NOTE -
    No reactor coolant pump (RCP) shall be started with any RCS cold leg
    temperature ≤ 350°F unless the secondary side water temperature of
    each steam generator is ≤ 50°F above each of the RCS cold leg
    temperatures and the RCP is started at ≤ 25% of rated RCP speed.

3.4.4 RCS Loops

  • NOTE -
    All RCPs may be removed from operation in MODE 3, 4, or 5 for ≤ 1 hour
    per 8 hour period provided:
    a. No operations are permitted that would cause introduction into the
    RCS, coolant with boron concentration less than required to meet
    the SDM of LCO 3.1.1; and
    b. Core outlet temperature is maintained at least 10°F below saturation
    temperature.

3.4.5 Pressurizer

A

Pressurizer Water level shall be ≤ 92%

APPLICABILITY: MODES 1, 2, 3, and MODE4 with all 4 cold leg temps > 275°F.

Initiate actions to fully insert all rods within 6 hours AND

3.4.6 Pressurizer Safety Valves

A

2 Safety Valves OPERABLE w/lift settings ≥2460 psig and ≤2510 psig

APPLICABILITY: MODES 1, 2, 3, and MODE4 with all 4 cold leg temps > 275°F.

  • NOTE -
    The lift settings are not required to be within the LCO limits during
    MODES 3 and 4 for the purpose of setting the pressurizer safety valves
    under ambient (hot) conditions. One pressurizer safety valve at a time
    may be inoperable for hot lift setting adjustment.
    This exception is allowed for 36 hours following entry into MODE 3,
    provided a preliminary cold setting was made prior to heatup.

    ACTIONS
    1 valve inoperable, restore valve to OPERABLE status within 15 minutes
    Required completion time above is not met or 2 valves are inoperable
    Be in MODE 3 within 6 hours AND

3.4.7 RCS LEAKAGE

A

RCS operational LEAKAGE shall be limited to:
a. No pressure boundary LEAKAGE,
b. 0.5 gpm unidentified LEAKAGE,
c. 10 gpm identified LEAKAGE from the RCS,
d. 150 gallons per day primary to secondary LEAKAGE through any
one Steam Generator (SG), and
e. 500 gallons per day primary to In-Containment Refueling Water
Storage Tank (IRWST) LEAKAGE through the passive residual heat
removal heat exchanger (PRHR HX).
APPLICABILITY: MODES 1, 2, 3, and 4.

Actions
A. Not within limits (other than pressure boundary or primary to secondary leakage) restore within 4 hours
Action A not met or pressure boundary or primary to secondary leakage exceeds limits, Be in MODE 3 within 6 hours AND
MODE 5 within 36 hours

3.4.8 Minimum RCS Flow

  • NOTE -
    a. All RCPs may be removed from operation for ≤ 1 hour per 8 hour
    period for the purpose of testing; or
    b. With no RCPs in operation, an unborated water source through the
    chemical mixing tank may be unisolated under administrative
    controls for ≤ 1 hour for the purpose of chemical addition to the
    pressurizer;
    provided:
    i. No operations are permitted that would cause introduction into
    the RCS, coolant with boron concentration less than required to
    meet the SDM of LCO 3.1.1; and
    ii. Core outlet temperature is maintained at least 10°F below
    saturation temperature.

3.4.9 RCS Leakage Detection Instrumentation

A

Following Instruments OPERABLE
2 containment sump level channels AND
The containment atmosphere F18 particulate monitor

  • NOTES -
    1. The following RCS leakage detection instrumentation is not required
    to be OPERABLE provided SR 3.4.7.1 is performed once per
    24 hours after 12 hours of steady state operation:
    a. The required containment sump level channels when
    In-containment Refueling Water Storage Tank (IRWST) gutter
    drain isolation valves are closed and for 2 hours after reopening
    IRWST gutter drain isolation valves; and
    b. The containment atmosphere F18 particulate monitor and
    required containment sump level channels when containment
    purge flow path is open and for 2 hours after containment purge
    flow path is closed.
    2. The containment atmosphere F18 particulate monitor is only
    required to be OPERABLE in MODE 1 with THERMAL POWER
    > 20% RTP.

A. One containment sump channel INOP, restore within 14 days.
B. Two containment sump channels INOP, restore one within 72 hours and perform leakrate 1x/24 hours
C. Particulate monitor INOP, Analyze grab samples of containment atmosphere 1x/24 hours OR
Perform leakrates 1x/24 hours AND restore the particulate monitor within 30 days.
D. Required actions of A, B. or C not met, Be in MODE 3 within 6 hours and MODE 5 within 36 hours.
E. ALL monitors INOP = 3.0.3

3.4.12 ADS - Shutdown, RCS INTACT

A

A. With reactor subcritical for < 28 hrs:
1. Five flow paths in ADS stage 1, 2, and 3 shall be OPERABLE;
and
2. Four flow paths in ADS stage 4 shall be OPERABLE
B. With reactor subcritical for ≥ 28 hrs:
1. Three flow paths in ADS stage 1, 2, and 3, with a minimum of
two flow paths in ADS stage 2 or 3, shall be OPERABLE; and
2. Three flow paths in ADS stage 4 shall be OPERABLE

APPLICABILITY: MODE 5 with RCS intact and pressurizer ≥ 20%.

ACTIONS
A. One flow path in ADS stage 1, 2, or 3 inoperable.
A.1 Restore to OPERABLE within 7 days
B. One flow path in stage 4 inoperable
B.1 Restore to OPERABLE within 72 hours

3.4.13 ADS - Shutdown, RCS OPEN

A

and LCO 3.4.13.

MODE 5 with RCS pressure boundary open,
MODE 6 with upper internals in place.

ACTIONS
A. One flow path in ADS stage 1, 2, AND 3 not open
A.1 Restore within 72 hours OR open alternative flow path with equivalent area within 72 hours.
B. One flow path in stage 4 inoperable
B.1 Restore to OPERABLE within 36 hours OR open alternative flow path with equivalent area within 36 hours
C. Required action/completion times of A or B not met in MODE 5 OR Condition A and B entered concurrently in MODE 5 or LCO not met for reasons other than A or B in MODE 5
C.1 Initiate action to fill the RCS to ≥ 20% pressurizer level AND suspended positive reactivity addition AND initiate action to VENT RCS IMMEDIATELY.
D. Required action/completion times of A or B not met in MODE 6 OR Condition A and B entered concurrently in MODE 6 or LCO not met for reasons other than A or B in MODE 6
D.1 Initiate action to remove upper internals IMMEDIATELY and suspended positive reactivity addition IMMEDIATELY

3.4.14 Low Temp Overpressure Protection (LTOP)

A

Following LTOP OPERABLE
TWO RNS suction relief valves and CVS makeup line containment isolation valve (CVS-V091) closed OR
the RCS depressurized and vented through ≥ 4.15 square inches.

- NOTE -
Accumulator isolation is only required when accumulator pressure is
greater than or equal to the maximum RCS pressure for the existing RCS
cold leg temperature allowed by the P/T limit curves provided in the
PTLR.

APPLICABILITY: MODE 4 with any cold leg temp ≤ 275°F, MODE 5 or MODE 6 with the head on

ACTIONS
A. Accumulator not isolated when pressure ≥ the maximum RCS pressure for existing cold leg temp per PTLR.
A.1 Isolate affected accumulator within 1 hour.
B. If RCS is not vented with ≥ 4.15 sq in hole and CVS-V091 not closed
B.1 Close valve within 1 hour
C Required action time of A not met
C.1 Raise cold leg temp or lower pressure to reach acceptable point on PTLR within 12 hours
D. LTOP INOP for reasons other than A, B, or C
D.1 Restore 2 RNS suction relief valves or depressurize and vent within 12 hours.

3.4.15 RCS Pressure Isolation Valve (PIV) Integrity

A

Leakage from PIVs shall be within limit.

APPLICABILITY: MODE 1, 2, 3, and 4 with RCS not cooled by RNS
ACTIONS
- NOTES -
1. Separate Condition entry is allowed for each flow path.
2. Enter applicable Conditions and Required Actions for systems made inoperable by an
inoperable PIV.
A. Leakage above limit
A.1 - NOTE -Each valve used to satisfy Required Action A.1 and Required Action A.2 must have been verified to meet SR 3.4.15.1 and
be in the reactor coolant pressure boundary or the high pressure portion of the system.
Isolate the high pressure portion from the low pressure by at least one closed manual, deactivated automatic, or check valve with 8 hours AND
A.2 Verify a second PIV can meet the leakage limits (valve required to be a check valve or closed valve if it isolates a line that penetrates containment) within 72 hours
B. Times not met, be in MODE 3 in 6 hours and MODE 5 within 36 hours.

3.4.16 Head Vents

A

3.4.17 SG Tube Integrity

A

SG tube integrity shall be maintained.
AND
All SG tubes satisfying the tube repair criteria shall be plugged IAW the Steam Generator Program.

APPLICABILITY: MODES 1, 2, 3, and 4.
ACTIONS
- NOTE -
Separate Condition entry is allowed for each SG tube.

A. One or more SG tubes satisfying the tube repair criteria and not plugged in accordance with the Steam
Generator Program.
A.1 Verify tube integrity of the affected tube(s) is maintained until the next refueling outage or SG tube inspection.
within 7 days
AND
A.2 Plug the affected tube(s) in accordance with the Steam Generator Program Prior to entering MODE 4 following the
next refueling outage or SG tube inspection.
B. Required Action and associated Completion Time of Condition A not met.
OR
SG tube integrity not maintained.
B.1 Be in MODE 3 within 6 hours
AND
B.2 Be in MODE 5 within 36 hours
