3.7.1 Main Steam Safety Valves

A

  • NOTE -
    The MSSVs are not required to be OPERABLE for opening in MODE 4 when the Reactor Coolant System (RCS) is being cooled by the Normal Residual Heat Removal System (RNS).

APPLICABILITY: MODES 1 - 4

ACTIONS
A. One or both steam generators with one or more valves INOP for opening
A.1 Reduce thermal power to less than or equal to maximum allowable per table within 4 hours AND
A.2 (In MODE 1) reduce OPΔT trip setpoint to less than maximum allowable per table within 36 hours
B. One or both steam generators with one or more valve INOP for closing, restore within 72 hours
C. Required action and completion time of A not met or One or both steam generators with ≥ 5 valves INOP for OPENING, BE in MODE 3 within 6 hours and MODE 4 with RNS cooling within 24 hours
D. Required action and completion time of B not met, MODE 3 within 6 hours and MODE 5 within 36 hours

  1. 7.2 Main Steam Line Isolation Valves
    a. Main steam isolation valves (MSIVs);
    b. MSIV bypass valves;
    c. Main steam line drain valves;
    d. Turbine stop valves or turbine control valves;
    e. Turbine bypass valves; and
    f. Moisture separator reheater 2nd stage steam isolation valves.

APPLICABILITY: MODES 1 - 4

ACTIONS
A. One MSIV INOP (MODE 1), restore within 8 hours
B. One or more turbine stop valve AND associated control valve, turbine bypass valve, or MSR 2nd stage isolation valve INOP (MODE 1), restore within 72 hours.
C. Two MSIVs INOP (MODE 1) OR One MSIV INOP and One or more turbine stop valve AND associated control valve, turbine bypass valve, or MSR 2nd stage isolation valve INOP OR required action and completion time of A or B not met, be in MODE 2 within 6 hours
D. One or two MSIVs or One or more turbine stop valve AND associated control valve, turbine bypass valve, or MSR 2nd stage isolation valve INOP (MODE 2 - 4)…ISOLATE affected flow path within 8 hours and verify isolated 1x/7 days.
E. One or more MSIV bypass or drain line valve INOP, isolate affected penetration by manual/blind flange, or closed deactivated automatic valve within 72 hours (may be unisolated under admin control) AND verify 1x/31 days
F. Required action and completion time of D or E not met, Be in MODE 3 within 6 hours, be in MODE 4 with RNS cooling within 24 hours and be in MODE 5 within 36 hours.

3.7.3 Main Feed Isolation and Control Valves (MFIVs and MFCVs)

APPLICABILITY: MODES 1 -4

NOTE -
Separate Condition entry is allowed for each feedwater flow path.

ACTIONS
A. One or both feedwater flow paths with MFIV or MFCV INOP, isolate flow path within 72 hours and verify it isolated 1x/7 days
B. One or both feedwater flow paths with MFIV and MFCV INOP, isolated flow path within 8 hours
C. Required action and completion time not met, be in MODE 3 within 6 hours, be in MODE 4 with RNS cooling within 24 hours and MODE 5 within 36 hours.

A

The specific activity of the secondary coolant shall be < 0.01 μCi/gm DOSE EQUIVALENT I-131.

3.7.5 Spent Fuel Pool Water level

A

Level ≥ 23 feet above top of irradiated fuel seated in storage racks

APPLICABILITY: When irradiated fuel is stored in the pool

ACTIONS
Less than 23 feet
- Suspend movement of irradiated fuel assemblies in the SFP IMMEDIATELY
- Initiate action to restore water level ≥ 23 feet within 1 hour

3.7.6 MCRE Habitability System (VES)

A

APPLICABILITY: MODES 1 - 4 and during movement of irradiated fuel

ACTIONS
A. One valve or damper INOP, restore within 7 days.
B. One PMS division INOP in one or more MCR load shed panel(s), restore within 7 days
C. Thermal mass of 1 or more heat sink(s) not within limits, restore required heat sink air temps within 24 hours and restore thermal mass of required heat sinks within 5 days.
D. VES INOP due to INOP MCRE boundary (MODES 1 - 4) IMMEDIATELY initiate action to implement mitigating actions AND verify mitigating actions ensure MCRE occupant exposures to rad/smoke/chemical will not exceed limits within 24 hours AND restore boundary within 90 days.
E. One bank of VES air tanks INOP,
E.1 Verify the OPERABLE tank has >245,680 scf of air within 2 hours AND 1x/12 hours thereafter AND
E.2 Verify VBS MCRE ancillary fans and supporting equipment are available within 24 hours AND
E.3 Restore VES to OPERABLE within 7 days.
F. Required action and completion time of A/B/C/D/E not met OR INOP for other reasons in MODES 1 - 4, MODE 3 within 6 hours and MODE 5 within 36 hours.
G. Required action and completion time of A/B/C/D/E not met or INOP for other reasons or INOP due to boundary during movement of irradiated fuel, SUSPEND movement of irradiated fuel IMMEDIATELY.

3.7.7 Startup Feedwater Valves (Control and Isolation)

NOTES -
1. Flow paths may be unisolated intermittently under administrative controls.
2. Separate Condition entry is allowed for each flow path.

A. One or more flow path with one valve INOP, Isolate affected flow path within 72 hours and verify isolated 1x/7 days
B. One flow path with two valves INOP, Isolate affected flow path within 8 hours
C. Required actions not met, be in MODE 3 within 6 hours, MODE 4 with RNS in service within 24 hours and MODE 5 within 36 hours.

3.7.8 Main Steam Line Leakage

A

Main Steam Line leakage through the pipe walls inside containment shall be ≤ 0.5 gpm.

APPLICABILITY: MODES 1 - 4

Leakage > 0.5 gpm, be in MODE 3 within 6 hours and MODE 5 within 36 hours.

3.7.9 SFP Makeup Water Sources

  • NOTES -
    1. OPERABILITY of the cask washdown pit is required when the spent
    fuel pool decay heat > 4.0 MWt and ≤ 7.0 MWt.
    2. OPERABILITY of the cask loading pit is required when the spent fuel
    pool decay heat > 5.0 MWt and ≤ 7.0 MWt.
    3. OPERABILITY of the Passive Containment Cooling Water Storage
    Tank (PCCWST) is required as a spent fuel pool makeup water
    source when the spent fuel pool decay heat > 7.0 MWt. If the
    reactor decay heat is > 7.0 MWt, the PCCWST must be exclusively
    available for containment cooling in accordance with LCO 3.6.6.
    4. OPERABILITY of the Fuel Transfer Canal is required.

A. One or more required makeup sources INOP, IMMEDIATELY initiate action to restore the makeup water source to OPERABLE

3.7.10 SG Isolation Valves

A

Each PORV/PORV block/SGBD isolation valve OPERABLE

APPLICABILITY: MODES 1 - 4

  • NOTE -
    PORV OPERABILITY is not required in MODE 4 with Reactor Coolant
    System (RCS) being cooled by the Normal Residual Heat Removal
    System (RNS).

  • NOTES -
    1. SG blowdown flow path(s) may be unisolated intermittently under administrative controls.
    2. Separate Condition entry is allowed for each flow path.

A. One or more SG PORV flow path with one valve INOP, isolate with closed deactivated automatic valve within 72 hours AND verify isolated 1x/31 days
B. One or SGBD flow paths with one valve INOP, isolate flow path within 72 hours and verify isolated 1x/7 days.
C. One or more SG PORV flow paths with 2 valves INOP, Isolate the affected flow path with at least one closed/deactivated automatic valve within 8 hours
D. One or more SGBD flow paths with 2 isolation valves INOP, Isolate affect flow path within 8 hours
E. Required action and completion time not met Be in MODE 3 within 6 hours AND MODE 4 with RNS in service within 24 hours and (no mode 5 for INOP PORV(s)) MODE 5 within 36 hours

3.7.11 SFP Boron Concentration

A

≥ 2300 ppm.

Applicability: When fuel assemblies are store in the SFP (3.0.3 is not applicable)

Spent fuel pool boron concentration not within limits, IMMEDIATELY suspend movement of fuel assemblies in the SFP AND initiate action to restore SFP BORON concentration IMMEDIATELY

ACTIONS

  • NOTES -
    1. Penetration flow path(s) may be unisolated intermittently under administrative controls.
    2. Separate Condition entry is allowed for each SFS penetration flow path.

A. One or more penetration with one or more SFS CIV INOP
A.1 Isolate the affect penetration within 24 hours AND
A.2.1 Isolate affected penetration with one closed deactivated automatic/manual/blind flange or check valve with flow secured within 7 DAYS or verify affected penetration isolated 1x/7 DAYS
B. Required action and completion time not met, declare IRWST INOP IMMEDIATELY
