What nerve are we aiming to innervate?
Inferior alveolar nerve
What is the theory of the block technique?
Placement of local anaesthetic solution around the main trunk of the sensory nerve or one of the major branches blocks all sensory info from the whole region of tissues supplied
What is the inferior alveolar nerve?
Sensory branch of mandibular nerve
3 red and last trunk of trigeminal nerve
Describe the placement of the id nerve
Passes through foramen ovale
Into infra temporal fossa
Between med and Lat pterygoid muscles
Behind lingual nerve
Through mandibular foramen and into mandibular canal
Runs forward and branches into incisive and mental nerve anteriorly
Where is the Idb achieved?
Mandibular foramen on medial aspect of Ramos before nerve enter the canal
What other nerve does it include?
Lingual nerve
What are the anatomical landmarks re. Idb placement?
Coronoid notch- external oblique ridge
Pterygomandibular raphe
What are contraindications re. Idb?
Haemophiliacs
Coagulation therapy therapy
Cooperation
What position should patient be in?
Supine
What is the pterygomandibular raphe?
Fibrous structure separating buccinator from superior constrictor muscle
Runs from Pterygoid halamus to mandible (retro molar)
Where should we aim to inject?
Raised ridge of mucous membrane which forms v shape with border of mandibular ramus
Where should the point of entry be?
From pre molars of opposite side
Within limbs of v
Half way up thumb
Above occlusal level of last standing molars
How much solution should be deposited?
2ml
Why should we aspirate?
To prevent intravascular injection
What indicates a successful injection?
Works rapidly
Tingling of lower lip
Tingling along lateral border of tongue
