A

A

Acute hyperkalemia (elevated potassium), acute hypocalcemia (decreased Calcium), Calcium channel blocker (nifedipine, verapamil, etc.)

Contraindications

A

Current digoxin therapy; Hypercalcemia; Hypersensitivity

Adverse effects

A

Bradycardia/ asystole arrest, hypotension, severe tissue necrosis if the is solution extravasation

Within seconds; peak effect 3-5 minutes; duration of 15-30 minutes

Adult dose

A

Hypocalcemia, calcium channel blocker OD, hyperkalemia and hypermagnesemia -
Calcium Choride 10% 10ml slow IV/IO over 2-3 minutes
Flush line before admin
May repeat in 10 min

Pretreat for IV/IO calcium channel blocker administration
300mg slow IV/IO
May repeat once

Pediatric dose

A

Hypocalcemia, calcium channel blocker OD, hyperkalemia and hypermagnesemia

Calcium Chloride 10% 20mg/kg slow IV/IO over 2-3 min
Repeat in 10 min if clinical indication persists (calcium channel blocker OD)
Max 1gm dose

Special considerations

A

Calcium Chloride is not recommended in patients with suspected digitalis toxicity

Use of smaller doses with patients taking Digitoxin, due to it possibly causing cardiac irritability

Rapid administration may cause bradycardia and hypotension; monitor blood pressure, heart rate and EKG
