Pre-Eclampsia?
What is it?
A
Multisystem disorder charcterised by four factors:
- Hypertension>140/90
- Occurs after 20 weeks
- Proteinuria >0.3g/24hours
- Nrmalises after delivery

Causes?
A
Extremes in age
Nulliparity
Multiple pregnancy
New partner
Past history of pre-eclampsia
High maternal BMI
Previous hypertension
Previous renal disease
Previous DM
Interval between pregnancies > 10 years

Pre-Eclampsia?
Symptoms?
A
May be asymptomatic
Headache
Visual disturbance
Abdominal pain (RUQ) or epigastric
Nausea and vomitting

Pre-Eclampsia?
Investigations?
A
Monitor fetal distress (CTG)
Bloods FBC, U&E, LFT, glucose screening for HELLP syndrome, uric acid level
BP
Urinalysis: Proteinuria
Neurology exam: hyperflexia, clonus,
Fundoscopy: papilloedema

Pre-Eclampsia?
Treatment?
A
Follow NICE guidelines:
- Conservative: Patient education/Regular BP monitoring
Medical:
- Labetalol 1st line
- Nifedipine and hyralazine
- Magnesium sulphate used to preven seizures

Fetal:
- Intrauterine growth restrictions
- Premature delivery
Maternal:
- Eclampsia
- HELLP syndrome
- Cerbral Hemorrhage
- Intra-abdominal hemorrhage
