What are some causes of primary hypothyroidism?
Hashimoto’s thyroiditis, primary atrophic hypothyroidism
What are some causes of secondary hypothyroidism?
Pituitary dysfunction, hypothalamic dysfunction
What is the world-wide leading cause of hypothyroidism?
Iodide deficiency
What are the key presentations of hypothyroidism?
Weight gain, depression, constipation, cold intolerance, brain fog
What are some signs of hypothyroidism?
Bradycardia, delayed reflexes
What would be the expected result of a TFT in primary hypothyroidism?
Increased TSH, usually decreased T3 and T4
What would be the expected result of a TFT in secondary hypothyroidism?
TSH inappropriately low, reduced T3 and T4
What drug would you prescibe?
Levothyroxine (Synthetic T4)
Who would you give a lower dose of levothyroxine to?
The elderly and patients with IHD
How would you monitor young and healthy patients?
Start on a high dose, review and adjust dose according to TSH, once TSH is normal check yearly
How would you monitor the elderly or patients with IHD?
Start on a low dose, increase dose by a little according to TSH
What are some complications of hypothyroidism?
Myxoedema coma, heart disease, pregnancy problems
What is myxoedema coma?
A medical emergency, the ultimate hypothyroid state before death
