Wells rule for DVT

A

1 point for each

  1. Active cancer
  2. Paralysis, bed ridden, immobility
  3. Bedridden for > 3 days, major surgery in 12 weeks
  4. Tenderness along DVT areas
  5. Calf swelling (>3mm asymmetry)
  6. Asymmetric pitting edema
  7. Collateral superficial veins
  8. Prior DVT

A

3 or more = high risk

1-2 = intermediate risk

0 or negative = low risk

12 Exclusion criteria for thrombolytics

A
  1. Intracranial hemorrhage on CT or history ever
  2. Clinical presentation of subarachnoid hemorrhage
  3. Neurosurgery, head trauma, or stroke in past 3 months
  4. Uncontrolled HTN (185/110)
  5. Known intracranial AVMs, neoplasm, or aneurysm
  6. Active internal bleeding
  7. Suspected/confirmed endocarditis
  8. Platelets < 100k
  9. Heparin in the last 48 hours with PTT elevated
  10. Current use of oral anticoagulatnts
  11. INR > 1.7
  12. Glucose < 50
