define pagets disease of the bone?
A
characterised by increased but uncontrolled bone turnover.

what is the aetiology of pagets disease?
A
- Excessive bone resorption by abnormally large osteoclasts is followed by increased bone formation by osteoblasts in a disorganised fashion
- This results in an abnormal pattern of lamellar bone

risk factors of pagets?
A
- being male
- old age
- family history

epidemiology of pagets disease?
A
- Common in the UK (5%)
* Only 1/20 patients are symptomatic

presenting symptoms of pagets disease?
A
- The skull, spine/pelvis and long bones of the legs are most affected
- May present with insidious onset bone pain
- Headaches
- Deafness
- Increasing skull size

signs of pagets on physical examination?
A
- Bossing of the skull
- Spinal kyphosis
- Bowing of the tibia

what will the bloods show for pagets disease?
A
o High ALP
o Ca2+ and phosphate = NORMAL

which resorption marker should you check?
A
o Check urinary hydroxyproline

what might a bone radiograph show?
A
SKULL CHANGES
- cotton wool appearance
- enlarged frontal and occipital regions.
