what is acapella?
- flow operated oscillatory device
what does the acapella use?
- uses a counterweighted plug and magnet to create airflow oscillations
what does the acapella help with? (2)
- loosens and moves secretions
what does acapella facilitate?
- facilitates the opening of airway in patients with secretory problems
what conditions may benefit from acapella? (6)
- atelectasis
- bronchitis
- bronchiectasis
- cystic fibrosis
- COPD
- asthma
what does acapella prolong?
- prolongs expiratory airflow to increase removal of secretion
what is the point of the inspiratory hold?
- encourages homogeneous ventilation
what position is acapella performed in?
- upright seating
- gravity assisted drainage position
what are the two colours of the devices?
- green
- blue
what is the green acapella used for?
- used for high flow >15L/min
what must the patient be able to do if they are using a green acapella?
- must be able to sustain at least 3 seconds of expiratory flow
what is the blue acapella for?
- for low flow <15L/min
what are the three indications of acapella?
- mucus producing respiratory conditions
- respiratory muscle weakness
- mechanical ventilation
how do you use the acapella device?
- take a deeper breath than normal inhalation through your nose with the lips sealed around the mouthpiece
- blow into the device with a forceful exhalation
where should you have your head when performing acapella? why?
- head tilted slightly forward to help keep the upper airways open
what should you monitor if someone is using an acapella? (4)
- dizziness
- light headedness
- chest discomfort
- desaturation
what should you be cautious of when using the acapella? (10)
- inability to tolerate increased WOB
- intracranial pressure > 20mmHg
- recent facial, oral or skull surgery/ trauma
- untreated pneumothorax
- haemoptysis
- haemodynamic instability
- nausea
- recent oesophageal surgery
- severe bronchospasm
- poor cognition or fatigue
