-MSO (medical service order) fill out and send with ff to dr/hosptial
-IAR (incident analysis report)
-Form F/A12 FF bill of right
-Employers report of occupational injury or illness (Form 5020)
- Workers compensation claim form (DWC 1)

Send the completed forms to
the Line BC for approval and signature. Line BC forwards completed forms to
Administration BC.
