Unit Commanders or their designated representatives will sign the DA 2765-1
Additionally, all walk-up requests must have signature of (S4) Budget Analyst, to verify funds. (The only exception to this policy is a walk-up requisition after duty hours.)

3) Packaged correctly to keep from being damaged while in storage
4) The item will coded batch code “B.”
If returning the item “Unserviceable”, the item must be and have the following documents: Under this circumstance the borrowing unit will be charged:
1) DD 2404 Technical inspection (TI) by ISE (as applicable)
2) DD Form 1577-2 Unserviceable Tag with authorized ISE stamped
3) Packaged correctly to keep from being damaged while in storage
4) Borrowing unit must furnish a new DA Form 2765-1 with a new document number to close out the temporary loan
