Anterior Uveitis
inflammation of anterior portion of uveal tract- iris and ant ciliary body
Panuveitis
inflammation of all uveal structures
Symptoms of Anterior Uveitis
pain
photophobia
tearing
blurred vision
Signs of Anterior Uveitis
perilimbal injection miosis- possible post synechiae keratic precipitates hypopyon or cells/flare in AC iris nodules or atrophy/ heterochromia high or low IOP
Extra testing needed for anterior uveitis when…
recurrent, severe, bilateral
chronic, granulomatous, resistant
suggest an underlying disease
Testing for Systemic Disorders Related to Anterior Uveitis
HLA-B27
blood tests
X rays- sacroiliac joint and chest
dilate for post seg inflammation
Tx for Anterior Uveitis
topical steroids- prednisone acetate 1%, taper
NSAIDs if long term
anti-metabolites
cycloplegic drops- pain relief and break up post synechiae- homatropine 5%
Spondyloarthropathy
joint disease of the vertebral column
HLA-B27 positive
Rheumatoid factor negative
Enthesitis
inflammation of ligament insertion into bone
early stage of ankylosing spondylitis
Ankylosing Spondylitis
eventual fusion of vertebrae - bamboo spine
starts as lower back stiffness/ pain
radiology shows changes to sacroiliac joint
HHLA B27 +
AAU in 25%
Reactive Arthritis
Reiter’s Syndrome
triad of urethritis, conjunctivitis, arthritis
HLA-B27 +
AAU in 20%
Psoriatic Arthritis
arthritis developing after psoriasis
AAU in 7%
Ulcerative Colitis
age 10- 30
lower abdominal cramps, bloody diarrhea
can evolve into colon cancer
AAU in 5%, esp if HLA-B27 +
Crohn’s Disease
granulomatous inflammation of intestinal wall
age 10- 30
abdominal pain and diarrhea
AAU in 3%
Tx for chronic AAU from spondyloarthropathies
systemic immunosuppresion
Fuch’s Uveitis Syndrome Presentation
chronic unilateral ocular inflammation of unknown cause hyperemia absent, eye white and quiet mild AC cells heterochromia- iris atrophy cataracts common develop glaucoma
Tx for Fuch’s Uveitis Syndrome
monitor for glaucoma
no steroids
Juvenile Idiopathic Arthritis
arthritis of unknown cause before age 16 that lasts more than 6 weeks
Ocular Complications of Juvenile Idiopathic Arthritis
band keratopathy cataracts glaucoma maculopathy- CME or ERM AAU
Mx of Juvenile Idiopathic Arthritis
exams every 3-4 months until age 12 topical steroids oral NSAIDs systemic immunosuppression if resistance Mydriasis to prevent post synechiae
Signs of AAU with Sarcoidosis
Busacca nodules- granulomatous bumps on iris
“mutton fat” keratitic infiltrates
Signs of AAU with Behcet’s Disease
hypopyon forms and dissolves rapidly and shifts with eye movement
Signs of AAU with HZV
sectoral iris atrophy- +TID
diffuse keratitic precipitates
Signs of AAU of HSV
linear endotheliitis
keratitic precipitates on endothelium
