Common anticoagulants

How it works
Prevents clot extension and formation.
Prevents DVT formation
Give to those with a high risk for thromboembolism

Indications
Prevention and treatment of thromboembolic disorders: DVT,PE, afib
Used in combo with thrombolytics and/or antiplatelet meds
Nursing Interventions
Assess for S/S bleeding: black tarry stools, haematuria, low h/h, unusual bleeding or bruising,
Assess PT/PTT, INR before administration
Patient teaching
Report signs of bleeding, do not take ASA or NSAIDS without discussing with MD, review high vit K foods, and encourage consistent intake of these foods

GInger,garlic, green tea, ginko
