Spironolactone, triamterene, amiloride
How it works
Inhibits the reabsorption of sodium in the kidneys while sparring potassium and hydrogen ions, lowers BP with weak diuretic effect
SE
Arrhythmia, SJS, toxic epidermal necrolysis, elevated potassium, decreased sodium
Indications
Used with other meds to manage HTN oR treat edema
Nursing Interventions
Assess BP before administration, I&O, daily weights, Monitor labs and for S/S of hyperkalemia, Monitor for skin rash, and periodic ECGs
Signs and symptoms of hyperkalemia
Fatigue, paresthesia, confusion, arrhythmia, dyspnea
Patient teaching
Notify if rash or muscle weakness, s/s of hyperkalemia, avoid salt substitution, reinforce other therapies for BP
