Dagdosis: geen AB
Opstoot van COPD

Verwekker: Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis, virussen
Dagdosis: augmentin 2 x 2g retard po ofwel amoxicilline/clavulaanzuur 4 x 1 g IV
opmerkingen: AB indien ofwel FEV1<50% en optstoot van COPD ofwel 2/3: toename dyspnee, toename sputa of purulent sputa
Duur: 7 dagen. Indien allergie of intolerantie voor beta-lactam AB of klinisch falen na 3 d moxifloxacine 400 mg po of moxifloxacine 400 mg IV
CAP
Verwekker: Streptococcus pneumoniae, Haemophilus influenza, Klebsiella spp, andere Enterobacteriacea, S areus
Dagdosis: amoxicilline/clavulaanzuur 4 x 1g IV. Zo meteen po R zo mogelijk: moxifloxacine 400 mg po
Opmerkingen: Indien allergie of intolerantie beta-lactam AB of klinisch falen na 3 dagen (ook atypische verwekker behandelen): moxifloxacine 400 mg po of moxifloxacine 400 mg IV
Duur: Indien M. pneumoniae en C. pneumoniae: 14 dagen. Indien Legionella: 14-21 dagen. Indien S pneumoniae: tot 72u koortsvrij. Standaard: 7d; 3-5 d voor lichte tot matige ernstige CAP en goede therapierespons
Overwhelming-CAP
Verwekker: S. pneumoniae, S areus of Legionella pneumophila
Dagdosis: ceftriaxone 1 x 2 g IV ofwel cefotaxime 3 x 2g IV telkens plus clarithromycine 2 x 500 mg IV
Aspiratiepneumonie
Verwekker: mond- keelflora
Dagdosis: amoxicilline/clavulaanzuur 4 x 1g IV
Primair longabces
Verwekker: anaërobe bct, Klebsiella spp
Dagdosis: amoxicilline/clavulaanzuur 4 x 1g IV ofwel cefuroxime 3 x 1.5g IV plus ornidazole 1 x 1 g IV
Duur: 2-6 weken
Secundaire multiple abcessen
Verwekker: S areus
Dagdosis: flucloxacilline 6 x 2 g IV
Pleura-empyeem
Verwekker: anaerobe bct, Streptococcus pneumoniae, S areus en Enterobacteriaceae
Dagdosis: amoxicilline/clavulaanzuur 4 x 1g IV
Opmerking: diagnostisch en therapeutische punctie/drainage
Duur: 2-6 weken
Nosocomiale pneumonie, vroegtijdig (< 7 dagen)
Verwekker: Streptococcus pneumoniae, H influenzae, Straphylococcus areus, Enterobacteriacea
Dagdosis: amoxiciline/clavulaanzuur 4 x 1 g IV
Nosocomiale pneumonie, laattijdig (> 7 dagen) of na voorafgaande AB-behandeling
Verwekker: enterobacteriacea, Staphylococcus areus
Dagdosis: ceftriaxone 1 x 2 g IV ofwel cefotaxime 3 x 2 gIV
Duur: indien initieel correcte therapie, geen immuunsupressie en goede klinische respons: 7 dagen
Nosocomiale pneumonie op INZO/MIC
Verwekker: Enterobacteriacea, Staphylococcus areus, Streptococcus pneumoniae, Pseudomonas aeruginosa
Dagdosis: piperacilline/tazobactam 4 x 4g/500mg IV ofwel ceftazidime 3 x 2g IV ofwel meropeneum 3 x 1 g IV telkens met of zonder amikacine IV
Opmerking: Initiele behandeling obv inventarisatie kweken en hospitalisatieduren en obv lokale epidemiologie en voorafgaande AB; steeds cultuur afnemen voor start van de behandeling. Indien P aeruginosa en andere non-fermenters: 14 dagen. Indien Staphylococcus areus: 10-14 dagen.
Nosocomiale pneumonie, bij immuungecompromiteerde pt
Initiële behandeling obv invasieve diagnostiek
Nosocomiale aspiratiepneumonie
Verwekker: mengflora
Dagdosis: piperacilline/tazobactam 3 x 4g/500 mg IV ofwel ceftriaxone 1 x 2 g IV ofwel cefotaxime 3 x 2g IV telkens plus ornidazol 1 x 1 g IV
Nosocomiale overwhelming pneumonie
Verwekker: enterobacteriaceae, Staphylococcus areus, Pseudomonas aeruginosa, Legionella species
Dagdosis: piperacilline/tazobactam 4 x 4 g/500mg IV ofwel ceftazidime 3 x 2g IV ofwel meropenem 3 x 1 g IV telkens plus clarithromcyine 2 x 500 mg IV met of zonder amikacine IV
Opmerking: indien vermoeden van MRSA: plus vancomycine 2 x 1g IV
Tuberculose
Verwekker: Mycobacterium tuberculosis
Dagdosis: isoniazide 1 x 300 mg po plus rifampicine 1 x 600 mg po plus pyrazinamide 1 x 2g po plus ethambutol 15mg/kg po
Opmerking: medicatie-inname niet spreiden, therapie aanpassen na twee maanden
