Differentials for ST elevation?
MI, ventricular aneurysm, pericarditis, normal variants, drug abuse
What is dressler syndrome?
when you get pericarditis awhile after having an MI
what is pulsus paradoxus?
A decrease in systolic BP of >10mmHg on inspiration
pericarditis on ekg
PR segment depressions, ST elevation, followed by Twave inversion
causes of pericarditis
CARDIAC RIND collagen vascular disease aortic dissection radiation drugs infections acute renal failure cardiac (MI) rheumatic fever injury neoplasms dresslers syndrome
treatment for DVT
Anticoagulate with IV unfractionated Heparin
or Subcutaneous LMWH followed by PO warfaren for 3-6 months.
treatment for pts with anticoagulate restrictions and they have a DVT
IVC filter
a very hi ankle brachial index may indicate
calcification of the artery

What does an S4 indicate?
Abnormal stiff ventricle or hypertrophic ventricle
what does an S3 indicate?
ventricular filling
what do you do when a person is having supra ventricular tach?
valsalva, carotid massage (if not contraindicated)
drugs of choice for supraventricular tacchycardia
adenosine and verapamil (but not until AFTER vagal manuvers of carotid massage and valsalva).
How does adenosine work?
It causes transient heart block of the AV node.
Also causes endothelial relaxation in the artery walls.
Tako tsubo cardiomyopathy
Acute myocardial damage most often occuring in postmenopausal women following an overwhelming emotionally stressful event. Leads to ballooning and left ventricular dyskinesis.
Mechanism massive catecholamine discharge.
Tx b blockers and acei
Treatment of pvc
No treatment. Even when associated with infarction. Treatment worsens the outcome.
