Patient on warfarin with stable INR expecting elective surgery
Stop warfarin for 2-3 days
Give vitamin K and proceed
Patient on warfarin needs an urgent surgery
Give fresh frozen plasma
But not for elective surgery as vitamin K can be used
WPW and ventricular tachycardia

Because they are often due to mediastinal malignancies which need to be ruled out first
