Chronic cough Flashcards
(20 cards)
Chronic bronchitis definition
A chronic cough that lasts for more than 3 months a year over 2 years
Clincal diagnosis
Causitive factor of chronic bronchitis
exposure to cigarette smoke
Other causes:
- Bacterial agents
- Influenza A/B virus
Pathology of Bronchitis (Chronic)
Mucous gland hypersecretion cause blockages in airway that plugs them. Obstructs air from getting out
Leads to increased CO2 and low O2
What do PFTs show in Chronic bronchitis
Show decreased FEV1, Decreased FVEV1/FVC ratio, normal or slightly decreased FVC.
Extremely decreased FEV1.
Sx of chronic bronchitis
- Obesity
- Cyanosis of lips
- Difficulty getting air out
- Crackles/Rales
- Tachypnea, dyspnea, hemoptysis, productive cough
Is chronic bronchitis associated with weight loss or weight gain
Weight gain– obese pts
Symptoms of chronic emphysema
- Barrel chest, Increased AP diameter
- Pursed lip breathing
- Can cause Pneumothorax as a complication
- Cachexia (wasting syndrome, lose weight)
- Wheezing
- Hyperresonance on percussion
Bullae on a CT are common in which disease
Emphysema
What are the symptoms of bronchiectisis
- Chronic cough with sputum production
- Hemoptysis
- Dyspnea
- Crackles, wheezing, clubbing (rare)
Bullae
no bronchial wall thickening
common finding in emphysema
Bullae
no bronchial wall thickening
common finding in emphysema
Bronchiectasis
Dilated bronchioles and bronchi and calcification around them
CAN ALSO SEE IN CF
Emphysema
Tree in bud apperance, Bronchiectasis
CXR finding of bronchiectasis
- Tree in bud apperance
- Wall thickening of bronchi
- Inflated bronchi
CF key pathophysiology
retain too much Na, salty skin
Infertility
Chronic cough with sputum, dyspnea, recurrent infections, pancreatic involvement w diabetes/ fatty stool
On exam CF findings
- Clubbing
- Cyanotic apperance
- Hyperresonance to percussion from air trapping
- Crackles
Key findings of CF lab results
ABG: Hypoxia
CT: Bronchiectasis
PFTs: Obstructive unless late stage
Blood Trypsinogen level: Elevated
CFTR shows gene dysfunction
Increased Na
Cystic fibrosis
Is CF obstructive or restrictive
Obstructive
