T-cell-mediated autoimmune disease (HLA DQ2) of small bowel

Reaction to gliadin (a prolamin (gluten protein) found in wheat)

Causing villous atrophy & malabsorption

Outline the typical presentation of coeliac disease

A
  • Any age (peak - infancy and 50-60yrs)
  • Stinking stools/ steatorrhoea
  • Diarrhoea & weight loss
    • Failure to thrive
  • Bloating abdo pain
  • Nausea & vomiting
  • Aphthous ulcers

How do you diagnose coeliac disease?

A
  • Antibody
    • a-gliadin
    • Transglutaminase
    • Anti-endomysial (IgA antibodies)
  • Endoscopy & duodenal biopsy
    • Villous atrophy
    • WBCs & crypt hyperplasia
  • Reverses on gluten free diet
