Visual Conversion Reaction (VCR) is a

psychosomatic anomaly

How did this patient present? (12 yr old)

- poor VAs
- severe color vision defect
- father says child is hyperactive
- no medication
- no family ocular hist
- doing poorly in 2 subjects at school (geo and reading)

What are the medical findings of the 12 yr old?

- Refraction with plano lens yield 20/20
- ocular and adnexa remarkable
- ocular motility jerky
- recognizes large color differences (not subtle)
- fails 4/6 green-red color testing

What is the next logical step that the researcher takes?

color testing done under hypnosis, patient is NOT color blind

What is the conclusion?

poor VAs due to VCR and color vision was a simulation of malingering for another gain. Referral to psychiatrist
