What should be noted in a prescription?
Signed in ink, dated, name and address of patient, address of prescriber, indication of type of prescriber, signature of prescriber, age and DOB of pt
1. Strength or quantity
2. More than a gram should be in grams, less than a gram should be milligrams
3. Micrograms/nanograms/units should not be abbreviated.
4. mL should be used, not cm3
5. Dose and dose frequency should be stated
6. Name of drug written clearly
7. Number of days of treatment
8. English without abbreviation (some Latin abbreviations)

Age and DOB of pt // name and address of pt
Amoxicillin oral suspension 125mg sugar-free
125 mg three times daily for 5 days
Supply 100ml

What should be included in a new patient exam?
Ask pt name, DOB, address
RFA, PCO, HPC
MH
FH, DH, SH
What pain history questions can you ask?
SOCRATES
- Can you point to the pain?
- When did you start feeling the pain? Was it sudden or gradual?
- can you describe the pain? Burning, stabbing, sharp, dull, throbbing?
- does the pain move to other parts of the body/face/jaw etc
- do you get any other symptoms with your pain?
- has your pain gotten better or worse since it started?
- does anything make the pain better or worse? Eg pain relief, hot/cold/sweet?
- on a scale of 1-10 how would you rate your pain?
- has the pain kept you up at night?
What medical history questions should you ask?
- problems with heart?
- high blood pressure?
- DVT or PE?
- blood disorders?
- breathing problems?
- diabetes, thyroid, endocrine issues?
- neurological conditions?
- stomach conditions?
- problems with or medications for bones or joints?
- kidneys or liver?
- autoimmune conditions?
- mental health conditions?
- HIV, hep B, TB?
- any other medical, physical or learning disabilities not previously mentioned
- operation or illness treated in hospital?
- cancer or radiotherapy to the head and neck region?
- you or your family ever had issues to LA or GA?
- ALLERGIES?
- prescribed medications?
- non prescribed drugs?
- steroid emergency card?
- smoke?
- alcohol
