Use an alcohol wipe with eyes closed to assess
olfactory nerve
Snellen eye chart / cover one eye, look at my nose, Check for superior & inferior peripherals vision
optic nerve
Dim lights watch pupils respond
oculomotor nerve
Move finger towards pt’s nose
Troclear Nerve
Use swab to check sensation at forehead, cheek & chin
trigeminal nerve
Look toward each ear, then six cardinal fields of gaze
abducens nerve
Have pt wrinkle forehead, puff cheeks, show teeth (look for symmetry)

Snap fingers, whisper
Acoustic nerve
Say “AHHHH” watch uvula
glossopharyngeal nerve
gag reflex
Vagus Nerve
Shrug shoulders, turn head side to side
spinal accessory nerve
Have pt stick tongue out
hypoglossal nerve
