Definition of cushing disease?

A

Where there is excess ACTH that comes from pituitary gland this is due to a pituitary adenoma

How does cortisol normally work in the body?

A

Pituitary gland releases ACTH which goes to adrenal gland and targets zona fasiculata, which then secretes cortisol

Cortisol levels are normally high in the mornings and low in the evenings and are kept normal by negative feedback.

A

Where cortisol levels are always high causing muscle and skin to break down and cause central obesity.

What causes cushing syndrome?

A

Exogenous ( outside factor)

Medications like steroids

Endogenous ( made by body)

Pituitary gland secretes too much ACTH which release more cortisol

Small cell lung cancer release ACTH

Tumour of adrenal gland cause increase ACTH production

How to diagnose Cushing syndrome?

A

Dexamethasone suppression test
Measure piss samples
ACTH plasma levels checked

Treatment for Cushing syndrome?

A

If caused by steroids withdraw them slowly

If a pituitary tumour a surgery should be done
