Dilated Cardiomyopathy

A

Path: Decreased contractility; virus, EtOH, ischemia, chemo
Pt: Systolic CHF: orthopnea, PND, DOE, crackles, dyspnea, JVD
Dx: Echo = dilated
Tx: CHF: BB, Ace-I, Diuretics.
Avoid/stop EtOH.
Avoid/stop chemo.
Transplant.

A
Path: genetics, sarcomeres
Pt: Murmur = Aortic Stenosis
Young Athletes
- Sudden Cardiac Death
- Syncope
- Dyspnea on exertion
Dx: Echo = Asymmetric
Tx: Avoid dehydration
Avoid exercise 
BB = CCB
EtOH Ablation, Myectomy
AICD for increased risk of death
Transplant

Path: HTN
Pt: Diastolic CHF
Dx: Echo = concentric 
Tx: DIA CHF
 -Avoid dehydration
 -CCB = BB
 -Control BLOOD PRESSURE

A

Path: amyloid, sarcoid, hemochromatosis, cancer and fibrosis
Pt: DIA CHF
Amyloid (right arrow) Neuropathy
Sarcoid (right arrow) Pulmonary disease
Hema (right arrow) Cirrhosis, Diabetes, CHF
Dx: Echo = Restrictive
Amyloid (right arrow) fat pad biopsy
Sarcoid (right arrow) cardiac MRI (right arrow) biopsy
Hema (right arrow) ferritin (right arrow) genetics
Tx: DIA CHF
-BB = CCB
Gentle Diuresis
Transplant
Underlying Disease
