Cholesterol Path
Path: need enough to make cells. Too much makes plaques. Plaques make atherosclerosis.
Pt: HDL is the good cholesterol
LDL is the bad cholesterol
Non-HDL cholesterol is what matters
Dx: Lipid Panel
Tx: Diet + exercise is always first; Adherence
F/u: Statins…..others
Who gets a Statin?
1) Vascular Disease (CVA, PVD, CAD)
2) LDL greater or equal to 190
3) LDL 70-189
+ Age 40-75
+ Diabetes
4) LDL 70-189

+ Calculated (risk factors)

Symptoms: Muscle soreness (CK); Hepatitis (LFTs)

Good effect: lower LDL, lower TG
Bad effect: Myositis, raise LFT
Fibrates
Good effect: lower total cholesterol, raise HDL
Bad effect: Myositis, raise LFT
Ezetimibe
Good effect: lower LDL
Bad effect: DIARRHEA
Bile-Acid Resins
Good effect: lower LDL
Bad effect: Diarrhea
Niacins
Good effect: raise HDL, lower LDL
Bad effect: Flushing, ASA
