(24 cards)
what is the next if the systolic is at least 180 or the diastolic is 120 and above?
Same day referral for accelerated HTN
What are some causes of secondary hypertension?
Renal artery stenosis
* Pheochromocytoma
* Aortic coarctation
* Cushing disease
* Primary hyperaldosteronism aka Conn’s Syndrome
* Hyperparathyroidism
* Renal parenchymal disease (glomerulonephritis, nephropathy, polycystic kidney disease)
* Alcohol
* Pregnancy
* Medications (oestrogen, NSAIDs, steroids)
What is Phaeochromocytoma?
- Rare tumor of the adrenal glands, specifically from the chromaffin cells
- Dysregulated release of adrenaline and noradrenaline
What are some symptoms of aortic coaration?
Radio-femoral delay, systolic ejection murmur, diminished lower extremity pulses,
differential upper and lower extremity BP
What are some symptoms of Phaeochromocytoma?
Anxiety or panic attacks, palpitations, tremor, sweating,
headache, flushing, nausea. Labile hypertension, tachycardia, bradycardia,
orthostatic hypotension
Name some complication with hypertension?
CAD
stroke
Chronic kidney disease
Peripheral artery disease
What is the pathophysilolgy of aortic dissection?
Atherosclerosis and constant exposure to high pressure leads to weakening of aortic wall
intimal tear blood flows into this space
* Mostly takes place in ascending aorta
What are the symptoms of aortic dissection?
Sudden, severe pain (tearing in character, interscapula
or anterior chest
* Nausea, vomiting, syncope, sweating
What are some side effects of diarectics?
ED
Gout
Hyponaterimia
Postural hypotension
Hypokalemia
Hyperglycemia
Rash
Polyuria
What is the diagnosis of Orthostatic hypertension?
Fall in systolic BP of at least 20 mmHg +/- fall in diastolic blood pressure of at least 10
mmHg
What are some causes of othrostatic hypertension?
Symptoms of othorstatic hypertension
Dizziness, weakness, blurry vision, lightheadness
What medications can be used to treat orthostatic hypertension?
Fludrocortisone, high-salt diet. Alpha-receptor agonists
(midodrine)
What are some possible cause of aortic reguration?
Rhematic disease
Infective endocardtis
Aortic dissection
Congential bicuspid aortic valve
What type of mumur is AR?
Mid- diastolic
What type of mumur is MS?
Mid- diastolic
Symptoms of aortic regurgitation
