Q

Should calcium carbonate be taken with food? Calcium citrate? What is the max dose to take due to absorption?

Calcium repletion pearls?

Hypercalcemia causes?

A

Yes. Doesn’t matter for citrate. 500 mg per dose.

Use calcium gluconate to reduce risk for extravasation. IV push in emergency ONLY.

Hyperparathyroid, malignancy, medications(thiazide diuretics, lithium, abuse of calcium containing antacids).

Q

Phosphorus repletion pearls?

What causes hyperphosphatemia?

Who to watch for with phosphate containing laxatives?

A

When doing IV if <4 mEq use potassium phosphate. If higher than 4 use sodium phosphate. Max infusion rate is 7.5 mMol/hr to prevent symptomatic hypocalcemia.

Kidney failure, rhabodmyolisis, vitamin d intoxication, phosphate containing laxatives.

Taking ARBS or NSAIDS or ACEi. Patiens >55, patients who are dehydrated,, patients with pre exisiting kidny dysfunction.
