2. Driver must show treating MD 3 months glucose records
3. Medical examiner must receive within 45 days of Form 5870 completion

Max Cert: 1 year
*may give 3 month certification if driver fails to bring 3 month glucose records

1. Unstable insulin regimen
2. DM not properly controlled

Question 3

ITDM or NIDDM Post-Severe Hypoglycemic Episode

Answer 3

• Driver must report to treating MD
• Driver cannot drive until MD determines cause and corrects it
• Treating MD must complete new Form 5870

Question 4

Disqualifying Severe Hypoglycemic Episode

Answer 4

1. Seizure
2. LOC or coma
3. Requires assistance from others

Question 5

ITDM and NIDDM DQ

Answer 5

1. Orthostatic hypotension
2. Peripheral neuropathy
3. Loss of position sensation
4. Loss of pedal sensation

Question 6

Certification of Driver with NIDDM Certification Period

Answer 6

Max Cert: 2 years

Same criteria apply for severe hypoglycemic episode
