Discharge Destinations
- Home Health
- Skilled Nursing Facility
- Sub-acute care
- Long Term Acute Care Hospital
Skilled Nursing Facility
Length of stay = 28 days
Therapies for patient = 1 or more /day (physical, occupational, speech)
Therapy length = 1 or 2 hours
- considered sub-acute a less intensive therapy
Acute Rehab
Length of stay = 16 days
Therapies for patient = physcial, occupation &/or speech
Therapy length = 3 hours/day
- patients here will benefit from an intensive multidisciplinary rehabilitation program
Long Term Acute Care Hospital
Average inpatient length of stay > 25 days “greather than a month”
- can exist within a hospital or stand alone “free-standing”
- provide care for patients recieving prolonged mechanical ventilation
When should PT recommendations for discharge following an acute care hospital stay be made?
1 - 4 days
