Describe how you would do this test

A

Position patient so they sit upright on bed so when supine, their head hangs over the edge
Explain to patient what you intend to do
Stand behind the patient, turn head 45 to 1 side
Support neck, lay patient flat in 1 quick movement
Make sure head hangs over end of bed
Ask patient to keep eyes open and observe for eye movement for 30s
Repeat with other side

Positive

  • vertigo experienced
  • rotatory nystagmus = superior semicircular canal involved
  • horizontal canal involved = lateral semicircular canal involved

Describe how you would do the Epley manouvre

A

Support neutral head hanging over bed edge in supine patient
30s pause
Turn head 30 to one side
Maintain this head position, ask patient to roll on their shoulder
Turn head so it faces floor, chin near shoulder
Keep for 30s
Gently bring patient back to sitting whilst keeping head in same position
Keep for 30s
Turn head to neutral and flex neck
Place chin on chest
Keep for 30s
