Emergency of the Day Flashcards
(31 cards)
- The aircraft departs during BFM while using an unusually high base height, what are your actions?
SPIN RECOVERY
- Comm?
- Ballistic vs Breakout?
- Flight path?
- Other formation members ht/eject call if still out of control
- Application of in/out spin aileron can create large side slip angles or even invert the spin and hence is prohibited
- It can take up to 5000 ft to pull out from the dive once recovery has been achieved
- Recovery achieved when:
◦Rotation stopped
◦Speed increasing
◦Aircraft responding normally to controls.
- Wx overcast at 300ft, Leuchars IFR, Runway 13.
At 90kts hear DC Bus Tie open
What are your actions?
ABORT
Considerations:
- Amend your T/O emergency brief to cater for the wx
- DC bus tie open vs Leuc transit? i.e. Low cloud base – Gen / HUD
- APU caption during formation wind up
APU FIRE
- Make radio call to ATC before egress
- Other ac move clear
- Warn other occupant if operating MDC
- Pairs T/O RW19 as #2, you see a flock of birds pass between the formation.
You abort at 130kts but are approaching the end of the runway, what are your actions?
BARRIER ENGAGEMENT
Considerations:
- Take off emergency brief?
- Formation considerations?
- Other aircraft actions?
- Barrier contract?
- Barrier Speed?
Leader has the barrier unless formally hands it over, will have to leave the runway to avoid the lead, consider ejection.
- Wx overcast at 700ft and stacked to 8000ft, RW31.
Just after getting airbourne and gear has been selected up you get an APU caption.
What are your actions?
APU FIRE
Considerations:
- 5000 ft ensures time to eject should the tailplane control rods burn through resulting in full ND deflection
- Use the mirrors to monitor for signs of fire (Ejection posture/Neg G)
2 reports part of the right main wheel spraying out from under the right wing.
- Leading a pairs T/O RW13. At 135kts just after rotate you hear a loud bang and feel the ac shudder.
What are your actions?
BLOWN TYRE ON TAKE-OFF
- If continuing T/O don’t put gear and flaps up.
- NB directional control issues if aborting.
- Climb above cct height before inspecting systems.
LANDING WITH A BLOWN TYRE
- You are on the chocks having suffered two HUMS failures.
During the 3rd start you get a fire caption.
What are your actions?
ENGINE FAILURES AND FIRES ON THE GROUND
then EGRESS
- Make sure seat is safe
- Tell ATC & ground crew
- During a GH sortie to the North of IOM, you are recovering from a final turn stall when the FIRE caption illumintes.
What are your actions?
NB: The caption goes out after undertaking the correct bold face.
FIRE
Considerations:
- Land ASAP - Divert
- Probable hot gas leak
- Keep using the mirrors to monitor for signs of fire (ejection posture)
- You are lined up RW 31 and cleared for T/O.
You select full power and get a T6NL caption.
What are your actions?
Considerations:
- Consider taxying clear of RW if caption clears on throttling back
- NB use low throttle RPM
- EEC not scheduling top end of the envelope
Possible EGRESS
- If caption clears, shut down and vacate as normal
- During engine start you turn on your avionics and start trying to set up as much as you can due to walking late.
You notice T6 rising quickly and approaching max for start.
What are your actions?
HOT START ON GROUND
Considerations:
- Tailwind on start?
- Ask trade support for advice.
- Flying a strafe profile you get an OIL caption.
What are your actions?
Considerations:
- Recover the aircraft
- Late arm
- G may cause further damage vs MSD
OIL PRESSURE - Nearest divs: St Athan vs Cardiff
- During a go around at Hawarden you experience a strong vibration from the engine followed by abnormal indications.
What are your actions?
ABNORMAL ENGINE BEHAVIOUR
Considerations:
- Fly away vs landing on?
- Min prac = full power if climbing away
- Fixed RPM will reduce loading on engine
- You are at ML in the airspace North of Prestwick
You get a T6NL caption, what are your actions?
T6NL/MFC/EEC
Considerations:
- Throttle most likely at mid power setting when caption illuminated - cannot second guess under/overswing
- 100% doesn’t necessarily mean full throttle
- RTB vs Transit?
- Prestwick may be the best option if wx is fit
- You are perfoming MPMs when you hear a loud bang and “surge, surge”.
What are your actions?
NB: The engine doesn’t respond to your initial actions.
Considerations:
- Recover the ac
- Immediate relight
- Point in sensible direction
- Tell wingman
- RTB due to unknown cause of engine issue
- You are on a BINGO profile from LFA 14 at FL330 after a short detachment.
After a short time you begin to feel nauseous and become aware of a slightly euphoric feeling.
What are your actions?
OXYGEN/HYPOXIA
Considerations:
- Squawk emerg to expedite descent
- Any Oxy snag should be auto diagnosed by the system
- BUOXY duration?
- You are in the latter stages of a spin setup when you become faintly aware of a strange smell.
Subsequently you notice a slight haze in your cockpit.
What are your actions?
COCKPIT SMOKE, FUMES OR FIRE
- Maybe coalescer sock in air con
- Strange smell is probably toxic
- You are transiting up to Leuchars at FL280 under radar control.
Shortly after passing abeam Carlisle you get an FPR caption.
What are your actions?
Considerations:
- Land ASAP
- Div key
- Radar control vs Altitude profile
- You are conducting ACM as the supporting fighter
After being threatened by the hostile you defend through the fight. While passing abeam the hostile you get HYD 1 and HYD 2 caption.
What are your initial actions?
Considerations:
- Fly the ac - clear flight path, KIO/LA safe
- Land ASAP - nearest?
- Hyd 2 cycles even with minimal inputs to controls
- Higher VApp for RAT efficiency
- Land with no NWS, chute and single application of brakes
- Will black runway.
- During a neutral BFM sortie you are becoming increasingly defensive during a flat scissors.
During the scissors you get a HYD 1 caption.
On recovering the ac you notice that you are unable to transmit.
When you left it was IRMs and overcast at 700ft, what are your actions?
HYDRAULICS 1
Considerations: - No comm KIO? - HEFOM Leading Considerations: - Difficult to slow <200kt without AB. Will need more airspace to position the formation - Stby gear & flap vs form recovery - Configure before cloud? - Land ASAP - nearest? - No NWS, chute, single application of brakes, black RW
- You are the lead of a pair in fighting wing through the lakes when you unavoidably fly through a flock of birds, you see them strike the aircraft.
Your engine indications appear normal.
What are your actions?
Considerations:
- Climb away from LL if wx allows (stay VMC)
- HEFOM to check systems
- Div key
CONTROLLABILITY CHECK - Configure near to div ideally on extended CL
- Landing Alpha/Speed
- Further emergencies may require land ASAP… Fuel, engine etc.
- Following a defensive BFM sortie you RTB and land on RW31.
No x-wind but you suddenly struggle to maintain directional control.
What are your actions?
LOSS OF DIRECTIONAL CONTROL
Consderations:
- Rudder effective down to 50 kts
- Brake chute - x wind
- Gear emerg retract not proven!
- You have just recovered on BINGO from D118.
On the break you select the gear down but do not get 3 greens.
What are your actions?
NB: Hyd pxs are normal
LANDING GEAR FAILS TO EXTEND
Considerations:
- Confirm button 1st
- CWP test ‘may’ latch on FIRE caption
- Stby gear lowering - unsafe config?
- On a GH sortie you roll from a cct at Hawarden when the gear does not retract.
What are your actions?
NB: Hyd 1 pxs are normal
LANDING GEAR FAILS TO RETRACT
Considerations:
- Indications are red illuminated gear handle only - do not know which gear(s) not retracted.
- High drag vs fuel consumption for RTB
- Hawarden LDA 6700 ft
- You are in an airway when you get a TRANS caption with associated audio.
What are your actions?
FUEL TRANSFER
Considerations:
- Radar control - get clearance to descend
- If fuel in centreline you will now have a C of G issue
- Total fuel is NOT usable fuel - delete CLT contents
- Land ASAPrac - div key
