A

11 beta hydroxy steroid dehydrogenase type 2

Cortisol has both glucocorticoid and mineralocorticoid actions. Cortisone is inactivated form with only gc action

Q

Actions of cortisol in cushings

A

1.protein - main
Protein catabolism… but actually mainly reduced protein anabolism in extrahepatic tissue. Aminoa cids uptake is reduced in extrahepatic tissues so proximal myopathy and increased in liver. So aa in blood increases, ie, alanine also increase in blood which is a main substrate for gluconeogenesis… taken up by liver.. thus increasing blood sugar level.

2.fat-mainly fat distribution and minimal lipolysis(which also increase blood sugar by gluconeogenesis

3.carohydrate - adrenal diabetes d/t lipolysis and alanine gluconeogenesis

4.mineralocorticoid action - triad : htn, alkalosis, hypokalemia

5.sex steroid action- acne, hirsuitism, amenorrhea/oligomenorrhea

6.cns - depression, steroid induced psychosis(rare)
