Pituitary Pars Intermedia Dysfunction (PPID)
AKA “Equine Cushings”

2. What does it do?

- Pituitary tumor in the par’s intermedia releases ACTH - stimulates adrenal glands to release cortisol.
- Increased cortisol interferes with insulin and many other body symptoms.
- Symptoms
- Long curly hair
- Excessive drinking
- Urination
- Laminitis
- Chronic infections
- High blood sugar
- Muscle wasting
- Poor BCS
- Abnormal fat deposits

- Pergolide- Dopamine Agonist “Prascend”
- Helps reduce the ACTH produced from the abnormal pituitary gland.
- $$$ - Maintain BCS w low starch & sugar
- Total diet w/less than 20% of these - Feeds that contain fat & fiber for calories while balancing nutrients is crucial
- Adequate protein to maintain muscle mass
- Added antioxidants - vita C & E
- Magnesium, Chromium, & Omega 3 fatty acids.
Insulin Resistance (IR)

- The diminished ability of cells to respond to the action of insulin in transporting glucose from the bloodstream into muscle & other tissues.
- Cause
- Associated with obesity or other health issue (cushings, BCS >7)
- Over feeding

- Low starch & sugar
- Total diet w/less than 20% of these - Adequate protein
- To maintain muscle mass along with balanced diet
- No starving if overweight! - Avoid large meals
- Feed small meals freq
- Nibble nets and slow feeders

Exertional Rhabdomyolysis (ER)

Clinical signs (4)
- Firm & painful muscles over the lumbar (loin) and sacral (croup) regions of the topline, including the large gluteal muscles.
- Excessive sweating, tachypnea, tachycardia, & muscle twitching are also noticed.
- Extreme cases: Reluctant/refuse to move
- Discolored urine due to release of myoglobin (muscle tissue damage) - Episodes may range from massive muscle necrosis & renal failure
Exertional Rhabdomyolysis (ER): Sporadic

- No prior Hx of tying-up episodes
- Single or very infeq episodes of ER - Causes
- Over-exertion relative to current level of conditioning
- Hyperthermia, dehydration, and electrolyte imbalances during endurance exercise
- Post-resp infection ?
Exertional Rhabdomyolysis (ER): Chronic

- Repeated episodes of tying-up from young age
- Increased serum creatine kinase (CK) and AST activities with mild exertion. - Causes
- Polysaccharide storage myopathy (PSSM)
- Recurrent exertional rhabdomyolysis (RER)
- Breeds: quarter horses, warm bloods, paints, appaloosas, morgans.
