Q

What are the subcutaneous mycoses?

A

Sporotrichosis, chromomycosis, mycetoma

17
Q

What causes sporotrichosis?

A

Sporothrix species, a thermally dimorphic fungi, enters the skin through a small injury like a thorn

18
Q

Manifestation of sporotrichosis?

A

Painless ulcer that spreads up lymphatics. If COPD- pulmonary. If immunosuppressed- meningitis or disseminated

19
Q

How do you diagnose sporotrichosis?

A

Biopsy for round or cigar shaped yeasts and culture pus at room temp for hyphae that look like daisies

20
Q

How do you treat sporotrichosis?

A

Oral azole, more serious form- amphotericin b

21
Q

What causes chromomycosis?

A

Tropical soil fungi enter through injuries to the feet

22
Q

Manifestation of chromomycosis?

A

Gradually spreading wart-like or plaque lesions with scattered black dots

23
Q

Diagnosis of chromomycosis?

A

KOH mount for gray or black septate hyphae. Biopsy dark spot for dark brown round fungal cells inside leukocytes or giant cells

24
Q

Treatment of chromomycosis?

A

Oral flucytosine, thiabendazole, local surgery, heat

25
What is mycetoma?
A rare infection of wounds on the extremities from Petriellidum or Madurella from the soil
26
What are the clinical manifestations of mycetoma?
Forms abscesses, granulomas, and pus with granules
27
How do you diagnose and treat mycetoma?
Biosy and treat with surgery, IV amphotericin B, and oral azole
