The 4 gram (-) rod pathogens
P. aeruginosa, B. cepacia, B. pseudomallei, B. mallei
P. aeuroginosa
Common and serious nosocomial pathogen
B. cepacia
Serious in context of CF
B. pseudomallei and B. mallei
Rare in U.S., but can be lethal
Common Qualities of these Pathogens
Gram (-) aerobes, nonfermenters, oxidase (+), grow easily in culture
Antibiotic resistant?
Yes, especially P. aeruginosa- through low permeability outer membrane and efflux pumps
Which two have minimal growth requirements and often contaminate hospital solutions?
P. aeruiginosa and B. cepacia
Community acquired presentations of P. aeruginosa
1) Endocarditis in IV drug addicts
2) Otitis externa in unchlorinated hot tubs
3) Osteochondritis in sneaker punctures
4) Corneal infections under contact lenses
P. aeruginosa presentation
Hospital
B. cepacia presentation
CF centers
B. pseudomallei presentation
Previously ill travelers/immigrants or Vietnam veterans
B. mallei
Previously ill travelers/immigrants with animal handling history
How can they all present?
Septicemia/pneumonia with poor prognosis
How to diagnose?
Culture and Gram stain
How to treat?
Latest combo of antibiotics. Test Ab sensitivity before and during treatment
